Sessions and events may be photographed or filmed by Everyone Plays CIC for promotional purposes, player feedback, and grant reporting. By ticking this box you consent to photographs or video in which you appear being used across our website, social media, and club communications. This covers content produced by Everyone Plays CIC only. We have no control over members of the public filming in public spaces. You can withdraw this consent at any time by emailing info@plymouthkickabouts.uk and we will remove your image from active use promptly.

I consent to my anonymised attendance data being used in grant applications, CIC annual reports, and impact reporting. My name will never appear in any report. Data is aggregated, for example: total sessions attended, age range, and postcode district only.

Every 6 weeks we send an optional short wellbeing survey (around 2 minutes). Results are fully anonymous and used only for internal monitoring and grant reporting. Ticking this box means we may send you the survey. You are never required to complete it.

I am the parent or guardian of the player named above. I consent to their participation in Everyone Plays CIC sessions and events, and to the data uses I have ticked above. I understand all communications regarding this player will be directed to me, not to the player directly, and that the player's name will not appear in any public communications.
